申請書類として求められるのは、基本的に以下の書類です。
（日本語での表記は簡略化しています）　 これらのほかに、申請者が必要と考える書類を提出してよく、逆に、追加書類の提出を求められる場合もあるようです（司法試験合格後に氏名が変わった場合などには追加書類の提出が求められることがあるかもしれません）。

（１）ニューヨーク州司法試験委員会からの合格通知
　（copy of the letter or notice of certification from the New York State Board of Law Examiners）
（２）質問票
　（application for admission questionnaire）
（３）道徳的人格を証明する宣誓供述書２通
　（two affidavits of good moral character）
（４）雇用の宣誓供述書又は書簡
　（employment affidavits or letters）
（５）弁護士登録されている法域での懲戒処分がなされていないことの証明書及び懲戒の書簡
　(certificates of good standing and grievance letters from jurisdictions in which you have been admitted to practice)
（６）ロースクールからの証明書
　(form certificates from your law school)
（７）代理人の指定書
（Designation of Agent）
